I already have an account. How do I log in?

Follow these steps:

• Enter your registered email address

What happens if I enter the wrong email address?

You will see an Invalid email error message. To resolve this:

• Make sure the email matches the one used during registration

• If you are unsure, contact support.

What happens if I enter the wrong password?

An Incorrect password error message will appear. To resolve this:

• Recheck your password and try again

• If forgotten, use the Forgot password? option.

Is my data secure on OpenMind?

Yes. OpenMind follows strict data protection practices:

• Your login credentials are encrypted

• Personal and health-related data is securely stored

• Data is not shared without your consent.

Why do I need to accept the Terms of Use?

The Terms of Use explain:

• How the platform works

• Your rights and responsibilities

• How your data is handled. You must accept them to access and use OpenMind services.

Can I view my password while typing?

Yes. To toggle password visibility:

• Click the eye icon next to the password field

• Click again to hide the password.

What should I do if I can’t log in?

Try the following steps:

• Check your internet connection

• Verify your email and password

• Reset your password if needed

• Restart the app or browser. If the issue continues, contact support.

02 Reset Password

How do I reset my password?

Follow these steps:

• Click Forgot password? on the login screen

• Enter your registered email address

• Click Send Reset Code

• You will be redirected to the reset page

• Enter your registered email address, the reset code received in your email, your new password, and confirm the new password

• Click Reset Password to complete the process.

What should I do if I don’t receive the reset code?

Try the following:

• Check your spam or junk folder

• Ensure you entered the correct email address

• Wait a few minutes and try again

• Click Send Reset Code again if needed.

Can I use any email address to reset my password?

No. You must use the email address registered with your account:

• Unregistered emails will not receive a reset code.

How long is the reset code valid?

The reset code is valid for a limited time (usually 10–15 minutes): If it expires, you need to request a new code.

What if I remember my password after requesting a reset?

No problem. You can cancel the reset process:

• Click Remembered your password? Login

• Log in using your existing password

• You do not need to complete the reset process.

I don’t have an account. How can I log in?

You cannot log in without an account. To get access:

• Visit the Contact Us page

• Submit your details and request account access

What is the Home Screen (Dashboard)?

The Home Screen is the main workspace of OpenMind where clinicians can:

What is the AI Scribe feature?

AI Scribe is an intelligent tool that helps you:

How do I select a patient?

Follow these steps:

• Click on the Select Patient dropdown

How do I select an appointment?

Follow these steps:

• This links documentation to a specific consultation.

What is the Template option used for?

The Template dropdown allows you to select a documentation format:

What are the Start, Pause, Resume, and Stop buttons?

These controls manage audio recording and transcription:

What is the Upload Audio feature?

The Upload Audio feature allows you to:

• Upload pre-recorded audio files

How do I use the AI chat box?

Follow these steps:

• Type your query in the Type here to chat with AI… field

• Click Ask

• The AI will provide assistance such as clinical suggestions, documentation help, and medical explanations.

What can I upload using the Upload File button?

You can upload: Patient reports

• Lab results

• Medical documents. The AI can analyze these files and assist in generating insights or summaries.

How do I upload audio using the Upload Audio button?

Follow these steps:

• Click on the Upload Audio button on the dashboard

• Browse and select the audio file from your device

• Confirm the upload

• The system will process the audio and convert it into transcription and clinical notes.

What is Chat History?

Chat History:

• Stores your previous AI conversations

• Allows you to revisit past queries and responses

• Helps maintain continuity in patient care.

What is the Chat History screen?

The Chat History screen displays all your previous AI chat sessions:

• It helps you review past conversations and clinical discussions

• Sessions are listed with date, time, and title.

How are chat sessions organized?

Chat sessions are sorted by creation date, newest first. The most recent session appears at the top of the list.

How do I view a previous chat session?

Follow these steps:

• Locate the session in the list

• Click the View button next to the session

• The full conversation will open for review.

What does the Chat Session Title represent?

The session title:

• Reflects the main topic or query of the conversation

• Helps you quickly identify the purpose of each session.

Can I filter chat history by date?

Yes. To filter by date:

• Use the Start Date and End Date fields

• Select the desired date range

• The system will display sessions within that period.

What happens if no sessions appear?

Try the following:

• Check if the selected date range is correct

• Ensure that chat sessions were previously created

• Try resetting or adjusting the filters.

What is the Items per page option?

This setting controls how many sessions appear on one page: You can adjust it (e.g., 10, 20) for easier viewing.

How does pagination work in Chat History?

To navigate pages:

• Use the Previous and Next buttons

• The current page number is highlighted

• This helps manage large numbers of chat sessions.

Can I continue a previous chat session?

Yes. Follow these steps:

• Click the View button next to the selected chat session

• You will see options: Copy Full Document, Save as Document, and Continue Chat

• Click Save as Document to download and save the file to your device

• Click Continue Chat to resume the conversation from where you left off.

What does New Chat do?

New Chat:

• Starts a fresh conversation with the AI

• Clears the current chat context.

What is the Close AI Scribe button?

The Close AI Scribe button ends and closes the current AI Scribe session.

What kind of support does the AI provide on the dashboard?

The AI assists in three main areas:

• Educational Support – Explains medical conditions and treatments; provides preventive care insights

• Symptom-Based Insights – Suggests possible diagnoses based on symptoms; supports clinical reasoning

What is the left-side navigation panel used for?

How do I log out of the dashboard?

To log out:

• Click Logout in the bottom-left corner

04 Add a New Patient

How do I add a new patient?

Follow these steps:

• Click on the Select Patient dropdown

• Select Add New Patient

• Enter the required details: First Name, Last Name, Email (optional), Date of Birth, and Gender

• Click Add Patient.

Which fields are mandatory when adding a patient?

The following fields are required:

• First Name and Last Name are required

• If left empty, an error message will be displayed

• Other fields like Email, Date of Birth, and Gender may be optional depending on system settings.

What happens if I don’t fill in required fields?

The system will display validation errors such as:

• “First name is required”

• “Last name is required”. You will not be able to add the patient until the required fields are completed.

Where can I see the added patient name?

Follow these steps:

• Click on the Select Patient dropdown

• View the list of available patients

• Select the required patient from the list

• Once selected, you can proceed to start the session or documentation.

Why is only the Start button enabled after selecting or adding a patient?

After selecting or adding a patient:

• Only the Start button is enabled by default

• Other controls like Pause, Resume, and Stop remain disabled until a session begins

• Click Start to begin recording or documentation.

How do the Start, Pause, Resume, and Stop buttons work during a session?

The session controls work in sequence:

• Click Start to begin the session

• Once started, the Pause button becomes enabled. Click Pause to temporarily stop

• After pausing, the Resume button becomes enabled. Click Resume to continue

• Once resumed, the Stop button becomes enabled. Click Stop to end the session and process the recording.

When can I view a patient’s report in Session Reports?

Reports become available after a completed session:

• Add or select a patient

• Click Start, conduct the session, and then click Stop to end it

• Once the session is processed, the report will be generated

• You can then view the report in Session Reports.

What does MindScribe do during a patient session?

AI Scribe listens to the conversation (live or uploaded audio), identifies key clinical terms, and automatically generates structured medical notes based on the selected template and view.

Why do I need to select a patient, appointment, and template before starting?

These selections ensure the documentation is linked correctly:

• Patient – Identifies who the note belongs to

• Appointment – Specifies which visit or session is being recorded

• Template – Defines the format and structure of the clinical note.

What is the purpose of different views like ADHD Diagnostic View or Symptoms View?

Views organize how information is captured and displayed:

• ADHD Diagnostic View – Focuses on behavioral and cognitive domains

• Symptoms View – Focuses on general symptom tracking. This helps tailor documentation to the clinical context.

How do pop-up questions work during recording?

What are Template Overview and Medical Condition Review?

06 Session Reports

What is the Session Reports section?

The Session Reports section allows you to view, search, and access previously generated clinical notes for patients.

What kind of reports are shown here?

It displays AI-generated session notes created using AI Scribe during patient appointments.

Who can access Session Reports?

Access depends on user roles and permissions (e.g., clinicians, admins).

How do I find reports for a specific patient?

Use the Search Patients field under Filter by Patient Name to quickly locate the relevant records.

Can I filter reports by date?

Yes. Use the date filter fields: Start Date – Set the beginning of the date range • End Date – Set the end of the date range.

What happens if I don’t apply any filters?

You will see all recent session reports by default.

Can I combine filters (patient and date)?

Yes. Combining filters helps you find very specific reports.

Why am I not seeing any results?

• No reports exist for the selected filters

How do I open a report?

How do I search for a specific document?

Can I filter summaries by date?

Yes. Use the date filter fields:

08 Create Template

What is the Create Template feature?

How do I create a new template?

Follow these steps:

• Navigate to Create Template

• Click Start Recording

• Dictate your template structure

• Click Stop Recording

What should I say after clicking Start Recording?

Clearly dictate the following:

• Template name

• Sections (e.g., Chief Complaint, History, Assessment, Plan)

Do I need to follow a specific format while dictating?

09 View Templates

Where are my created templates saved?

All templates you create are automatically saved in the View Templates section under All Templates.

Can I edit a template after creating it?

No. Templates cannot be edited after creation. You would need to create a new template if changes are required.

What is the difference between Custom and POD templates?

There are two template types:

• Custom Templates – Created by you or your organization

• POD Templates – Predefined system templates provided by the platform.

How do I use a template for a patient session?

Follow these steps:

• Select a patient

• Select an appointment

• Choose a template from the Template dropdown

10 Casebook & Patient Management

How do I create a new patient casebook?

Follow these steps:

• Click the + Create Patient Casebook button at the top right of the Casebook screen

• Fill in the required fields: First Name, Last Name, Email, Date of Birth, and Gender

• Optionally set an Appointment date and time

• Click Create Casebook to save.

What is the difference between Today’s Appointments and All Patients tabs?

There are two tabs:

• Today’s Appointments – Shows only patients who have a scheduled appointment for the current day

• All Patients – Shows your complete list of registered patients, regardless of appointment status.

How do I search for a specific patient?

Use the Search by Patient Name field at the top of the Casebook screen. Type any part of the patient’s name to filter results in real time.

Can I view patients in different layouts?

Yes. You can switch between Table View (a structured list with columns for Patient ID, Name, Age, Gender, Created, and Last Updated) and Card View (a visual card-based layout) using the toggle buttons at the top right.

How is a Patient ID assigned?

Patient IDs are automatically generated in the format OMD-YYYY-NNNNN (e.g., OMD-2026-00005). You do not need to assign them manually.

Can I sort the patient list?

Yes. On the All Patients tab, use the Sort by dropdown to sort by options such as Last Updated (Newest First) or other available criteria.

11 Patient Files & Documents

What types of files can I upload to a patient’s casebook?

You can upload diagnostic reports, genetic tests, imaging results (such as CT scans or MRIs), and other medical documents. PDFs are fully supported.

How do I upload a file for a patient?

Follow these steps:

• Open the patient’s casebook by clicking their name in the list

• Go to the Patient Files tab

• Click the Upload File button

• Select your file. Once uploaded, it will appear in the Patient Files table with the date, document name, description, and category.

What are AI Summaries?

After uploading a document, OpenMind’s AI can automatically analyze it and generate a concise clinical summary. Click View AI Summaries next to any document in the Patient Files table to read the AI-generated summary.

Can I search or filter patient documents?

Yes. Within the Patient Files section, you can filter documents by Document Name, Start Date, and End Date to quickly locate specific records.

What can the AI assistant do?

The AI assistant can help you:

• Ask clinical questions about a specific patient based on their uploaded files

• Summarize patient history, symptoms, medications, and assessments

• Generate evidence-based treatment plans tailored to the patient

• Run structured clinical workflows to support decision-making.

How do I ask the AI about a patient?

Follow these steps:

• Open the patient’s casebook and go to the Patient Files tab

• Type your question in the Ask about [Patient Name]… text box

• Press Ask to receive an AI-generated response based on the patient’s files and data.

What is Treatment Planning?

Treatment Planning is a built-in AI workflow that generates personalized, evidence-based treatment plans for a patient. It takes into account the patient’s uploaded files, clinical notes, and summary data.

Can I view past AI conversations for a patient?

Yes. Click the Chat History button in the patient’s casebook to review all previous AI conversations associated with that patient. Use New Chat to start a fresh session.

13 Assessment & Prescribe Test

How do I prescribe a cognitive assessment test to a patient?

Follow these steps:

• Navigate to Assessment > Prescribe Test

• Select the required assessment test

• Click Next

• Search for and select a patient

• Click Next to review the details

• Click Prescribe Test to send the assessment invitation.

What tests are available under Prescribe Test?

Available assessments are displayed on the Select Test screen:

• Tests may vary based on your organization’s configuration

• Example: Creyos Cognitive Assessment.

How do I select a patient for a test?

Follow these steps:

• Complete the Select Test step

• Click Next

• Search for the patient using the search box

• Select the checkbox beside the patient name

• Click Next to continue.

Can I search for a patient before prescribing a test?

Yes. To search for a patient:

• Use the Search Patient field

• Enter the patient’s full or partial name

• Matching records are displayed instantly.

What information is displayed in the patient list?

Each patient record shows:

• Patient Name

• Age

• Gender

• Email Address.

Can I add a new patient while prescribing a test?

Yes. Follow these steps:

• Click + Add Patient

• Enter the required details: First Name, Last Name, Email, Date of Birth, and Gender

• Save the patient record

• Select the newly added patient and continue.

Can I review patient details before prescribing the test?

Yes. The confirmation screen displays:

• Test Name

Can I edit patient details before sending the test?

• Click Edit on the confirmation screen

• Update the patient selection if needed

What happens after I click Prescribe Test?

The following occurs:

• An assessment invitation email is sent to the patient

• The email contains a secure assessment link

• The patient can complete the test using the provided link.

How will I know if the test was successfully prescribed?

You will receive the following confirmation:

• The system displays a success confirmation message

• The assessment invitation is sent to the patient’s registered email address.

Can I view completed test results?

Yes. Follow these steps:

• Navigate to Assessment > View Test Reports

• Open the patient’s report

• Review the assessment results and findings.

What happens if no patient is selected?

If no patient is selected:

• The Next button remains disabled

• A patient must be selected before proceeding.

Can I prescribe a test to multiple patients at once?

No. Each test prescription is assigned to one patient only: Repeat the process for additional patients.

What should I do if the patient does not receive the assessment email?

Try the following:

• Verify the patient’s email address

• Ask the patient to check their Spam or Junk folder

• Resend the assessment if required

• Contact support if the issue continues.

Can I cancel the prescription process?

Yes. To cancel:

• Click Cancel at any step

• No invitation will be sent until Prescribe Test is clicked.

What happens if there are no patients available in the list?

If the list is empty:

• Click + Add Patient to create a new patient record

• Select the newly created patient and continue.

Can I go back and modify my selections?

Yes. To make changes:

• Use the Back button to return to previous steps

• Update the selected test or patient

• Continue with the revised information.

When will the test results become available?

Results become available after:

• The patient completes the assessment

• Reports can then be viewed in View Test Reports.

14 Set Your Preferences

What is the Set Your Preferences screen?

The Set Your Preferences screen allows you to customize how AI Scribe generates clinical documentation before starting a session. It includes six configurable settings: Template, Specialty, Report Length, Format, Pop-out Controls, and AI Clinical Supervisor.

How do I access the Set Your Preferences screen?

Follow these steps:

• From the AI Scribe dashboard, click on Preferences in the bottom-left navigation

• The Set Your Preferences panel will open with all configurable options on the left side

• Select each setting, make your choice, and click Save to apply.

What templates are available to choose from?

The Template section offers the following options:

• SOAP Report – SOAP format for structured clinical documentation

• Medical Documentation Template – Standard medical documentation template for clinical use

• Post-Operative Follow-Up Note – Template for post-operative patient follow-up documentation

• Post-Operative Note – Structured post-operative documentation template

• Empty Template – Basic template structure for medical documentation. Additional templates may be available by scrolling the list.

Which template is selected by default?

SOAP Report is selected by default, as it is the most commonly used format for structured clinical documentation.

Can I change the template before starting a session?

Yes. You can select any available template from the Set Your Preferences screen before starting. It is recommended to choose your template prior to beginning a session, as changing it mid-session may affect report consistency.

What specialties are available under the Specialty setting?

The following specialties are currently available:

• Psychiatry

• Neurology

• General (selected by default). Additional specialties may be added in future updates.

What does selecting a specialty do?

Selecting a specialty helps the AI tailor the terminology, structure, and clinical insights in the generated report to match that specific medical domain. For example, selecting Psychiatry will orient the output toward behavioral and mental health documentation.

Which specialty is selected by default?

General is selected by default, making it suitable for a broad range of clinical documentation needs.

What are the Report Length options?

There are three report length options:

• Concise – A short summary focused on the most important clinical points

• Standard – A balanced clinical document with appropriate detail (selected by default)

• Detailed – A comprehensive report with in-depth clinical insights and expanded documentation.

Which report length should I choose?

Choose based on your documentation needs:

• Use Concise for quick follow-up visits or brief consultations

• Use Standard for routine appointments

• Use Detailed for complex cases requiring thorough documentation.

Which report length is selected by default?

Standard is selected by default, providing a balanced level of clinical detail for most session types.

What are the Format options?

The Format setting controls how the AI structures the report output:

• List – Organized in a structured, point-wise format

• Paragraphs – Written as narrative clinical notes in prose form

• Bullets – Presented as easy-to-read bullet points (selected by default).

Which format is selected by default?

Bullets is selected by default for easy readability and quick scanning of clinical notes.

What are Pop-out Controls?

Pop-out Controls determine what additional interface elements appear in the AI Scribe pop-out window during a session. This setting has three options: Disable, Audio Controls, and Audio Controls + Pop-up History.

What are the Pop-out Controls options?

There are three options:

• Disable – No pop-out controls are shown (selected by default). The AI Scribe interface stays within the current screen

• Audio Controls – Displays Start, Pause, Resume, and Stop recording controls in the pop-out window

• Enable – The AI reviews and refines the generated documentation for improved clinical accuracy • Disable – The AI generates raw output without the additional review layer (selected by default).

• Review each preference section: Template, Specialty, Report Length, Format, Pop-out Controls, and AI Clinical Supervisor

• Click the Save button at the bottom right of the screen to apply your selections • Click Cancel to discard any changes and keep the previous settings.

Can I change my preferences at any time?

Yes. Preferences can be updated before starting a session by returning to the Preferences screen. Some settings may not take effect once a recording session has already begun.

Do preferences apply to all future sessions?

Preferences are saved and applied to subsequent sessions. Some platforms may retain your last-used selections as the default for future sessions.

What happens if I don’t configure any preferences?

If no preferences are set, the system applies default settings automatically: SOAP Report template, General specialty, Standard report length, Bullets format, Pop-out Controls disabled, and AI Clinical Supervisor disabled.
